Check a note
Paste a note and you get back what it does not establish, with the sentence that would. The condition is worked out from the note itself. Accept the ones true of your patient, take the corrected note, and generate the RFA.

Remove the patient’s details before you paste
Take out the name, date of birth, medical record and claim numbers, address, phone number and email. The clinical content is what gets checked — the findings, the durations, the imaging — and none of it needs a name attached.
The note is read once to answer the checklist questions and is not stored. Files are opened in this browser; only the text is sent.
We check for identifiers as soon as you paste, but this is a spot-check, not a guarantee — it looks for things with a recognisable shape, like a date of birth or a record number. It cannot spot a name written into a sentence. Please read it over yourself.
